Please complete this form before or by 7:50 a.m. the day of the absence, if your child is sick or will otherwise be absent from school.  The school office may contact you to verify details.  The school will require communication from parents or guardian within 24 hours to verify the reason for a student absence. Excused absences include funerals, illness, injury, legal obligations, medical procedures and religious observations, and extenuating circumstances determined by the principal. The school may require suitable proof regarding the above exceptions, including written statements from medical sources. These written statements may be emailed to Shelby Smith at Shelby.Smith@jeffco.k12.co.us or delivered to the Patterson International's office. Schools will then notify the parents or guardian if the absence will be considered unexcused pursuant to district policy. Any absence not specifically covered by another section, such as family vacations, college visits, or other extended absence, must complete a pre-arranged absence form in the school office in order to be excused. This form must be submitted at least three days prior to the scheduled absence.
